notified in writing prior to May 23, 2009. Requests
for refund after May 23, 2009 must be of an emergency nature and in writing to the Board of Directors
from agency head. Any refunds processed after May 23, 2009 will be subject to a $100 processing fee.
Any requests after the conference must be received no later than July 29, 2009.
